Are you PASSIONATE about caring for people? SYNERGY HomeCare of the Lowcountry (“SYNERGY”) is looking for an individual to join our team as a CARE COORDINATOR. At SYNERGY, we provide services to people of all ages, providing only the best companionship, homemaking and personal care services throughout Beaufort and Jasper counties. Our caregivers are responsible for companionship (conversation, planning activities, medication reminders, maintaining a safe environment, hospital sitting, etc.), homemaker activities (light housekeeping, grocery shopping, running errands, meal preparation, etc.), and personal care (bathing, grooming, lifting, etc.).
Job Description
The Care Coordinator is expected to perform a variety of duties, working closely with the Scheduling Coordinator in order to provide the highest quality service to our Care Staff and the Clients we service. This position is an ON-CALL position which requires you to answer phones from your home or remotely from the hours of 5:00 pm to 8:00 am Monday- Friday and on the weekends. Note that this is a rotating position, so you will not be required to work every week. If you are interested in working overnights during the week, or on weekends, or both, we are interested in talking to you.
Responsibilities:
- Review schedules with the Scheduling Coordinator
- Answer all incoming phone calls and respond as needed
- Communicate with Caregivers and Clients as schedule changes arise
- Communicate regularly with Caregivers
- Confirm upcoming shifts
- Maintain list of standby Caregivers for evenings and weekends
- Understand Caregiver availability
- Document all calls and contact backup Caregiver if necessary
- Update scheduling software system with any schedule changes
- Other items as needed
Qualifications:
- Be PASSIONATE about caring for people
- Possess excellent computer skills
- Exhibit professional phone etiquette
- Demonstrate an ability to multitask and prioritize
