Support Advocate National Center for Health Equity's (ANCHE) Teammate Social Determinants of Health (SDOH) Help NOW & Emergency Care Fund Lead in establishing the strategy and related programming to assist teammates with urgent non-clinical needs. Assist the SDOH Lead in designing ANCHE’s strategy and targeted interventions to improve the well-being of Advocate Health teammates through leading and strengthening Advocate Health’s Help NOW teammate social impact program.
Provides oversight and guidance to Advocate Health Community Health Workers providing 1:1 assistance on urgent non-medical needs to teammates across the Advocate Health enterprise.
REMOTE POSITION
MAJOR RESPONSIBILITIES
- Provide content expertise and recommendations on solutions for teammate non-medical urgent needs, with special emphasis on food insecurity, housing affordability and homelessness prevention, safety, employment and economic mobility, digital inclusion, and basic economic needs.
- Assist Advocate Health teammates and strategic external partners by providing advice and guidance on accessing established and emerging Advocate Health social impact programs and related opportunities.
- Serve as the primary point of contact for community-based organizations and agencies outside of Advocate Health working to improve social conditions in communities where Advocate Health provides care, including especially in priority social impact areas related to food insecurity, affordable housing, safety, career development, digital inclusion, and basic economic needs.
- Provide direction, guidance, and oversight for Advocate Health Community Health Workers providing teammates 1:1 assistance with urgent, essential, non-clinical needs.
- Oversee the Help NOW’s digital presence, including its connection and coordination with the Find Help online referral network, as well as Help NOW resources available to teammates via internal channels (including PeopleConnect).
MINIMUM EDUCATION AND EXPERIENCE REQUIRED
Education
- Bachelor's Degree; Advanced Degree preferred.
- Field of study may include public health, epidemiology, social work, or a related in the social or behavioral sciences
Years of Experience
Requires a minimum 2 years of direct experience addressing and improving the social drivers of health for underserved populations.
MINIMUM KNOWLEDGE, SKILLS AND ABILITIES
- Experience developing and managing programs and partnerships with community-based organizations.
- Demonstrated experience addressing the social drivers of health; previous work engagements addressing access to food, affordable housing, and safety for underserved populations desirable
- Knowledge of current best practices in large health systems impacting non-clinical social drivers of health
- Competitive candidates will possess an appreciation and understanding of the literature related to the social drivers of health, especially the impact of housing, food, and safety on overall health and well-being
- Excellent communication and listening skills
- Proven project management skills
